Content and distribution of asbestos particles in the lung of mesothelioma
    Takumi Kishimoto,MD,PhD

For 143 case of malignant mesothelioma(135 cases of male, 8 cases of female), the number of asbestos particles are counted by the modified method of Kohyama. The age ranged from 31 to 89 years with the median of 67 years old. One hundred thirty one cases originated from pleura and 12 cases from peritoneum. For the pathological types of tumors, 52.4% showed epithelioid type, both biphasic and sarcomatoid were 20.3% , 2.8% of desmplastic type and the other 14.2% are histologically unknown.
For the histories of asbestos exposure, 86.4% of them had occupational exposure, 0.7% environment exposure and 10.5% had non-asbestos exposure and other 2.1% were unknown. Fifty six point six percent exhibited pleural plaques on the chest x-ray or CT scanning and only 0.7% showed asbestosis.
The content of asbestos particles in the 1g of dry lung tissue ranged from 35 to 1,740,957. Fifty point three percent of them had more than 5,000 particles and 18.9% had less than 1,000. and other 30.8 % had between 1,000 to 4,999 particles. About 80% had occupational asbestos exposed histories and more than 1,000 asbestos particles.
Furthermore, the distribution of asbestos particles in the laterality or distribution of upper, middle or lower bodies of lungs were also examine for 44 cases of mesothelioma. Upper lobes contained 133,215±229,784 particles with the median of 10,832, middle lobes 81,058±116,115 with median of 12,999 and lower lobes of 100,915±180,721 with the median 19,656 particles, which meant no significant distributions among upper middle, and lower lobes of lungs. Furthermore, laterality of lungs, upper partilces per lower particles in the left side was 1.0±0.6 with the median of 1.2 and the right side was 1.5±1.6 with median of 1.0. These data suggested that deposit of asbestos particles in the lung had no literalities nor distribution of lungs
Eight seven point one percent of mesothelioma in Japan were induced by the occupational exposure to asbestos and 81.1% of mesothelioma cases had more than 1,000 asbestos particles per 1g of dry lung tissue. On the other hand, the laterality and distribution of the deposit of asbestos particles in the lung tissues were not observed by the counting asbestos particles.
